Off Pump Coronary Artery Bypass OPCAB

During off-pump coronary artery bypass (OPCAB) the patient's myocardial contractions are slowed but not stopped. This procedure doesn't use a cross-clamp however it does require the use of a partial occlusion clamp.

Partial occlusions clamps, often called Kay-Lambert or Satinsky clamps, are used to create a bloodless field in order to perform the proximal anastamosis of a vessel graft to the aorta. This can be performed while the heart is beating in an OPCAB (Off Pump Coronary Artery Bypass) procedure.

  • Surgeons typically choose to use metal clamps because of the slippery nature of the vessel, especially when it is beating.
  • Metal clamps can damage the intima of the aorta and break pieces of plaque becoming lodged in smaller vessels downstream.
